Important info for registering as a new patient

Below is a step-by-step guide to our registration process..

1. If you are a new patient, you can apply to join the practice by emailing hello@templebardoc.com and informing us that you are hoping to register
   

2. The manager will send you out a registration form. 

3. Once you have filled the form fully and return it to us via email or in-person, we will then set you up with your first appointment, and this is triaged.

NOTE: There is a waiting time for new patients for the first consultation of up to a month or more depending on the time of year, and this new-patient waiting time is to facilitate our existing patients.

Once you are registered, you will be seen same day for urgent care, or next available  booked appointment.

If you are coming in for abortion services or cervical smear, you can bypass this waiting list by booking directly as abortion and / or cervical check patients so you do not need to go on the waiting list.
